Custom Personal Health Support is a collection of programs and services that identify at-risk employees of large clients for early interventions and provides support to improve their health and productivity while reducing costs. Available to groups 5,000+ ASO only.

Personal Health Support 2.0

Personal Health Support 2.0 is a proactive care management solution designed to improve employee health and drive down benefit costs.  It addresses 50+ conditions and provides outreach to 100% of the members in an employer’s population who have identified gaps in care. Available to groups 750+ ASO only.

Standard Care Coordination

Standard Care Coordination (SCC) provides members who are facing a health situation with much needed support through health care professionals, clinical programs and information. Standard Care Coordination encourages wise health care decisions by providing employees with expert advice and clinical resources.

Healthy Pregnancy Program

The Healthy Pregnancy Program is a personalized maternity wellness program that provides members with additional support and education throughout their pregnancy. We work closely with expectant mothers to identify potentially high-risk pregnancies, refer them to Neonatal Resource Services, if purchased, high-risk case management, and access to Centers of Excellence network for quality and cost–efficient care.

Reproductive Resource Services

Infertility programs help reduce costs associated with infertility through clinical consulting and guidance to infertility treatment centers with superior outcomes. This helps reduce both treatment and downstream costs associated with multiple gestation pregnancies. Available for groups 3,000+ both ASO and FI.

Neonatal Resource Service (Neonatal Intensive Care)

The Neonatal Resource Services program offers onsite and telephonic case management across the nation.  There are onsite Nurses within 140 facilities in over 30 markets.  NRS helps control and reduce neonatal care costs with effective onsite and telephone case management that guides parents to high quality and efficient facilities. 

QuitPower

QuitPower is our tobacco cessation program. QuitPower provides members with an effective means to quit tobacco and improve their health care through a personalized program, a dedicated wellness coach and Nicotine Replacement Therapy (NRT) if needed. The program focuses on specific measurable and attainable goals, such as establishing a quit date and avoiding triggers that lead to tobacco use. Available for groups 750+ ASO only.

Cancer Resource Services

Cancer Resource Services offers information and member assistance through a team of experienced cancer nurse consultants. They are available to help individuals understand their own or a loved one’s cancer diagnosis, its implications and possible treatments. Availble for ASO only.

Healthy Back Program

The Healthy Back Program targets members with low back pain, the largest musculoskeletal health care cost category. It can help deliver substantial savings by improving self-care and care-seeking patterns for individuals with recurring and chronic back conditions. Available for groups 750+ ASO only.

Standard Wellness Coaching

Standard Wellness Coaching is a lifestyle program that supports and reinforces changes to help improve weight, stress management, exercise, nutrition, tobacco cessation, heart health and diabetes. The objectives of Standard Wellness Coaching are to help members identify behaviors that need to be changed, set personalized goals and focus on correction of those behaviors. Available for groups 750+ ASO only.

Worksite Wellness

Worksite Wellness is a suite of worksite wellness solutions that may help employers decrease absenteeism, increase productivity, improve morale, improve employee retention and reduce employee health care expenditures over the long term by providing health services to members at the worksite or in their homes. Available for ASO only groups.

Cancer Support Program

Cancer Support Program is a comprehensive, integrated single source to help manage cancer-related costs and to help improve the quality of care and quality of life for your employees and their dependents. Available for groups 3,000+ ASO only.

Congenital Heart Disease Resource Services

Congenital Heart Disease Resource Services allows access to the nation’s top Congenital Heart Disease centers and provides extensive information to patients to assist them in selecting a center for their care.

Kidney Resource Services

The Kidney Resource Services Program provides information to educate and guide members to top performing dialysis clinics and reduce End Stage Renal Disease (the final stage of Chronic Kidney Disease) -related medical expenses.  The program provides a comprehensive set of services through a team of specialized nurses who help patients make informed treatment decisions and improve their health care experiences.

Transplant Resource Services

Our Transplant Resource Services help employers provide high-quality transplant care at an affordable cost. UnitedHealthcare uses the best clinical centers in the country, as identified by the Clinical Sciences Institute (CSI) – a panel of over 170 independent, practicing transplant surgeons who annually review and qualify the best clinical facilities.

24-Hour Health Concierge

UnitedHealthcare NurseLineSM is a 24/7 health advocacy solution. It delivers greater value than typical triage-only services through increased program referrals, a unique call-back model, member engagement techniques and more. Members are connected with a single point of contact, a health care expert, who guides them to the right resources.

Diabetes Management Program

The Diabetes Management program seeks to reduce unnecessary hospitalizations and health care costs and works to improve the health of those individuals who have diabetes. Available for ASO only.

Bariatric Resource Services

Bariatric Resource Services can help reduce complications associated with weight loss surgery for morbid obesity. Not available in all markets for FI clients.

HealtheNotes

HealtheNotes are personalized, health opportunity messages sent to members (and their physicians) that identify health improvement and medication savings opportunities. They are delivered by mail or online through the Health & Wellness website. Available to groups 100+ ASO only.

HealtheNote Reminders

HealtheNote Reminders are designed to remind members to consult their physicians and health care professionals for recommended routine preventive care. The reminders are delivered via mail to a member’s home and can reach up to 30% of an employer’s member population. 

Treatment Decision Support

Treatment Decision Support (TDS) program helps members actively engage in treatment decisions for their conditions by providing them information about their medical conditions and possible treatment options to discuss with their health care providers, and the clinical and cost ramifications of their treatment choices. Available to groups 750+ ASO only.

Emergency Room Decision Support

The ER Decision Support program targets members before they become frequent ER users and it addresses underlying reasons members use the ER inappropriately, such as lack of knowledge of appropriate resources, including a Primary Care Physician (PCP). Available to groups 750+ ASO only.

Asthma Management Program

Asthma is a prevalent, and growing, costly health problem for Americans.  The Asthma disease management program helps reduce unnecessary hospitalizations and health care costs, missed days of work and seeks to improve the individual’s quality of life. 

Coronary Artery Disease (CAD)

Our Coronary Artery Disease program is designed to help reduce unnecessary hospitalizations and health care costs, and improve quality of life. Through sophisticated stratification, members are targeted for inventions based on acuity level and potential for impact. Available for ASO only.

Chronic Obstructive Pulmonary Disease Management Program (COPD)

Our program provides education, self-management skills and can explain treatment options that may be available to an individual, based upon the recommendation of their health care provider. Available for ASO only.
